Redden Modified Ultimate (Each) The Redden Modified Ultimate™ was designed by Dr. Redden as an emergency aid for treating acute laminitis. The Modified Ultimate significantly reduces the action of the deep flexor tendon which anatomically is a direct antagonist to the laminae. Significantly reducing the opposing forces transferred via the coffin bone greatly enhances the healing environment, reduces the risk of digital displacement (rotation and sinking), and helps diminish clinical pain. Dealing with literally hundreds of high scale damaged cases (scored clinically and radiographically), Dr. Redden has concluded that optimum mechanical properties are obtained when the Ultimates provide a 20 degree palmar angle with the ground forces. For additional information, See the related items below. ![]() Note: When the palmar angle exceeds 5 degrees, the heels must be pushed back reestablishing a 0 - 3 degree palmar angle, this assures heel loading, shifting internal load from the apex. Precise breakover and derotation can only be accurately assessed with pure lateral soft tissue detail film. Dr. Redden takes a scout film and another set of laterals once the Ultimate is on the foot. Once the Ultimates are applied the laminae is never tested, not even for a few seconds for the duration of the recovery. Future trimming, radiographs or routine exam should be non weight bearing, quick and atraumatic. Loading the foot without the mechanical protection greatly increases the risk of displacement and unwarranted damage to the laminae. How to wean a horse out of the Ultimates: First, you must fully appreciate the degree of damage to each foot. This can be determined via consistent, repeatable, soft tissue, pure lateral radiographs, venograms and clinical observation. The slightest change in soft tissue perimeters indicates a significant insult has occurred. Venograms can offer insight to previous damage and the degree of present damage. The clinical response to pain is not a valid, dependable indicator and should not be used to assess the stability of the syndrome. Once the horse has been weaned off all anti-inflammatories, remains clinically sound for 10 - 15 days, has no noticeable digital pulse and no radiographic displacement, e.g. horn lamellar zones normal and sole depth unchanged or thicker, one wedge can be removed. Continue stall rest for an additional 10 - 15 days, repeat the film and clinical exam, and remove the last wedge if all parameters are normal. Once the normal foot has recovered from a significant bout (approx. 30-45 days) and the Ultimate is removed, Dr Redden applies a four point shoe or four point trim when indicated, applying breakover well behind the apex of PIII for an additional 30 days. Depending on the goals, athletic commitment, and economic consideration, it is possible for many to proceed with a slow, easy training or turn-out at this stage of recovery. The depth and size of the 6 week growth ring can be a realistic indicator for sport or speed horses. All should remain asymptomatic without the benefit of anti-inflammatories during the first few months back in training. Note: Many very significant cases can be successfully treated in this fashion and have full recovery. It is common though for many significant insults to require 6 - 12 months for full recovery.
